Store Description: Purchase of office supplies (pens, paper, etc.)
Amount: $150
Authorized by: [Your Name or the person responsible for approving the transaction]

Once you’ve filled out the cash voucher, you’d attach the receipt from the office supplies store to the voucher as a supporting document.
